A-Team (Care Coordination)

Growing Strong Together Work Group (Children Youth and Family)

Community Resource Work Group (Behavorial Health)

 Interdisciplinary team of care and social service providers from different agencies in the community who coordinate care for people with complex health and social needs. 


Meetings:

Every Tuesday from 3-4:30pm

Community Resource Work Group (Behavorial Health)

Convene cross-sector representation to identify needs, brainstorm solutions and increase access to address behavioral health, to include substance use and mental health, in Kittitas County.  


Meetings:

Every 3rd Thursday of every month

from 12 to 1:30 pm

 Collaborate with youth-serving organizations to understand what resources are available, address emerging needs, and support childcare providers and Kittitas County youth     


Meetings: 

 Every 4th Wednesday of each month from 3 pm to 4 pm

    The Leadership Council is comprised of cross-sector agency representatives and community members who are highly engaged in leading community health improvement activities.

Health Equity

 Collaborate with network partners, community organizations, and healthcare providers to identify health disparities and inequities, to integrate health equity into all agencies, and create a fair and just environment for all Kittitas County residents.  


Meetings:

 Quarterly, Every 3rd Tuesday (in February, May, August and November) at 1 pm
