Care Coordination & A-Team

Behavioral Health Work Group

Behavioral Health Work Group

Responsible for oversight of planning, data collection, performance measures, and implementation activities for Strategic Issue #1 and associated goals and strategies. Overseeing implementation of Health Commons care coordination technology and establishing future vision of care coordination services in Kittitas County.

 A-Team 

 Team of care providers from different agencies in the community who coordinate care for people who use emergency services frequently or are at risk of re-hospitalization.

Behavioral Health Work Group

Service providers and community members interested in opportunities to  improve behavioral health services, increase interagency communication and expand the coordination of care. The work group promotes "warm handoff” referrals, and “no wrong door” policies through assistance with system and resource navigation. 


Meetings:

Every 2nd Thursday at 12 pm

Resilience & ACEs Collaborative Work Group

Kittitas County Jail.

Resilience & ACEs Collaborative Work Group

The work group is driven by cross sector collaborations to educate the community on ACEs and Resilience. The goal is to move from a trauma blind to a more trauma informed community to build supports and resilience.

Every 3rd Wednesday 1pm to 2pm

Group of data-minded individuals from partner organizations to provide data and performance measure supports to KCHN efforts. This work group is essential in the development of the Community Health Assessment and the Community Health Improvement Plan.

 The CCWG is driven by the Strategic Plan created from the findings of the Childcare Needs Assessment that was completed in 2021. The group works in collaboration with providers, leaders, parents, families, and members of the community to focus on bringing solutions to the unmet need of child care in Kittitas county.  


Meetings:

Every 3rd Thursday at 1 pm
