Care Coordination

Behavioral Health Work Group

Behavioral Health Work Group

Responsible for oversight of planning, data collection, performance measures, and implementation activities for Strategic Issue #1 and associated goals and strategies. Overseeing implementation of Health Commons care coordination technology and establishing future vision of care coordination services in Kittitas County.

Behavioral Health Work Group

Service providers and community members interested in funding opportunities that improve behavioral health services, improving interagency communication and improving the coordination of care for people with behavioral health needs through assistance with system navigation, “warm handoff” referrals, and “no wrong door” policies. A 2019 priority is refining a proposal for a 1/10 of 1% sales tax for behavioral health to take to the Kittitas County Commissioners.

Group funded by a HRSA planning grant to complete provider and community education, a needs assessment, strategic plan, workforce plan, and sustainability plan around opioid use disorder prevention, treatment, and recovery.
