 Step 1: Apply for DDA Eligibility 

 KCHN Community Health workers can guide and assist with the online application process on the 

DDA website. 

 Step 2: Request Services 

 If approved, a KCHN Community Health worker will follow the instructions from the Plan Action Notice to request services on the DDA Services and Request 

web form. 

 The disability must: 

• Start before the person turns 18.
• Continue or be expected to continue for life.
• Pose a substantial limit to the person.

 A disability associated with:  

• Intellectual disability. 
• Cerebral palsy.
• Epilepsy.
• Autism.
• Any neurological or other condition closely related to an intellectual disability or that requires support like individuals with intellectual disabilities.

• Personal Care
• Relief Care
• Employment Support 
• Skills Acquisition Training 
• Specialized Clothing
• Specialized Equipment and Supplies 
• Aggregate Services (e.g., positive behavior supports, skilled nursing care, home and vehicle modifications, staff & family training).
