Hearts & HAnds: A Service Provided by KCHN

Now operating through the Care Coordination Program, we provide peace, comfort and support to the most vulnerable population in Kittitas County. This includes those suffering from a chronic illness, disability, the frail, elderly, and those with short term medical needs. We provide these services to anyone in Kittitas County, regardless of income,

free of charge.
